How do I register with the practice –?
If you are interested in registering with this practice, you will need to either download an application form or alternatively collect one from the practice.

What do I do if my personal details such as moving house, surname etc changes?
Please call the practice if it is a change in address, if you are changing your surname then we will need some proof, perhaps a marriage certificate.

Can I have a home visit?
If your condition means you cannot come to the surgery, you can be visited at the doctor’s discretion. Please telephone your request before 10.00am and give the receptionist as much information as possible to help the doctor assess the urgency of your call. You may be telephoned by the doctor to discuss your request.

Can I view my medical records?
Please pick up a leaflet from the surgery, which explains how you can go about requesting to view your medical records.

How do I contact my Health Visitor
Please telephone the Surgery on 01536 791300 and ask for Audrey Spence – Health visitor or leave a message with the receptionist and Audrey will return your call.

How do I order a repeat prescription?
You may email, fax or drop a repeat request into the black letter box outside the building or alternatively you may use our online email facility.

How do I access service out of hours
Please telephone the surgery first on 01536 791300 and listen to the Out of Hours instructions alternative if you are calling between 6:30pm and 8:00am and at the weekends and Bank Holiday, please contact the Out of Hours service directly on 03336 664 664

How do I make an appointment?
To make an appointment please telephone the surgery on 01536 791300. Please remember if you want to see a specific GP you may have to wait a little longer.
