How do I register with the practice?
If you are interested in registering with the practice, you will need to download an application form – see ‘How to Register’ on our Home page. Alternatively, you can collect the registration form from the practice.

What do I do if my personal details such as moving house, surname etc changes?
If you change your surname, we will need to see some legal proof of your name change such as a marriage certificate etc. If you change your address and it is outside of our practice boundary, then you may need to register with a GP nearer where you live. For other minor changes such as your telephone/mobile number, this can now be done online if you are registered for online services. 

Can I have a home visit?
If your condition means you cannot come to the surgery, you can be visited at the doctor’s discretion. Please telephone your request before 10.00am and give the receptionist as much information as possible to help the doctor assess the urgency of your call. You may be telephoned by the doctor to discuss your request.

Can I view my medical records?
Please pick up a leaflet from the surgery, which explains how you can go about requesting to view your medical records.

How do I contact my Health Visitor
Please telephone the Surgery on 01536 791300 and ask for Ann Binnie or Claire Collier, or leave a message with the receptionist and one of the Health Visitors will return your call.

How do I order a repeat prescription?
We must have all requests for prescriptions in writing. Please register to use our online services if you are taking medication on a regular monthly basis as this is the best way of re-ordering your medication. If you do not have access to the internet, then please fax or complete a request and drop it in the letter box. 

How do I access services after you have closed?
Please telephone the surgery first on 01536 791300 and listen to the Out of Hours instructions. Alternatively, if you are calling between 6:30pm and 8:00am and/or at the weekends and Bank Holidays, please contact the Out of Hours service directly on 111.

How do I make an appointment?
To make an appointment please telephone the surgery on 01536 791300. Please remember if you want to see a specific GP you may have to wait a little longer. Alternatively, a limited number of appointments are available online. If you are registered for online services you will be able to book an appointment online.
